Coming Soon: Changes to Department of Aging Provider Certification Process

Due to upcoming changes in the provider certification process, applications to be certified as an agency, non-agency (business entity), or assisted living provider must be completed by April 1, 2021.  Applications to be certified as a non-agency (non-business entity) or participant-directed provider must be completed by April 15, 2021.  Providers who do not complete their applications by these dates must wait until July 26, 2021 to reapply. Learn more about changes coming soon to the certification process.
