How do I apply for a Golden Buckeye card?

If you are age 60 or better, you may not have to apply. Cards are sent automatically to Ohio residents in the month of their 60th birthday. We use records from the Ohio Bureau of Motor Vehicles for these mailings, so you must have a current Ohio Driver License or ID Card with your correct birth date and address to receive your card automatically.

If you have a license or ID card, but your information is incorrect, call toll-free 1-866-301-6446 during normal business hours (Monday thru Friday, 8:00 A.M. to 5:00 P.M.) to provide the correct information and request your card be re-sent.

If you do not have a Driver License or state ID card, (or if you do, but did not receive your card for unknown reasons) you can apply for a Golden Buckeye card at most senior centers and public libraries. Please bring with you proof of age.

If you are under age 60 and qualify for a Golden Buckeye card due to disability, you can apply for one at most senior centers and public libraries. Please bring proof of age and certification of disability.

We cannot accept applications or requests for Golden Buckeye cards online or by phone.

Call toll-free 1-866-301-6446 during normal business hours (Monday thru Friday, 8:00 A.M. to 5:00 P.M.) to verify your mailing information. You may also apply for a card at most senior centers and public libraries. If you apply in person, you'll be given a temporary card that you can use right away.

I applied or called for a new or replacement Golden Buckeye card but have not yet received it. What should I do?

You should receive your permanent card in six to eight weeks after your application is faxed to us. If you have not received your card in this time period, call toll-free 1-866-301-6446 during normal business hours (Monday thru Friday, 8:00 A.M. to 5:00 P.M.) to confirm that your application has been received and processed.

How old do you have to be to get a Golden Buckeye card?

To be eligible for a Golden Buckeye card, you must be an Ohio resident age 60 or older OR an adult resident (age 18 to 59) with a permanent and total disability (as defined by social security).

I thought the age for a Golden Buckeye card was 55. When did it change?

The minimum age of eligibility for a Golden Buckeye card has been 60 since 1985. Prior to that (from the program's launch in 1976), the minimum age was 65. From 1985 to 1988, the Golden Buckeye card partnered with the Southwestern Bell Silver Saver card, which was available to folks 55 and older, but the age of eligibility for Golden Buckeye was still 60.

What will you accept as proof of disability for a Golden Buckeye card?

To prove eligibility for a Golden Buckeye card due to disability, we need one of the following documents:

  • a Medicare (not Medicaid) card;
  • SSDI acceptance documentation; or
  • a copy of a document certifying permanent and total disability issued by any state department, Ohio public employee retirement system (STRS, PERS, etc), or any federal retirement system.

We are unable to accept any other form of proof of disability, such as a letter from your doctor.
