Care Options
The Ohio Department of Aging believes that everyone has the right to receive the care they need in the settings they prefer. The aging network helps Ohioans understand their options for care and ensures your rights as a consumer are not infringed.
Help at Home - Many Ohioans need assistance with care, but don't necessarily need to go into a nursing home or other facility. They just need some help and support to remain at home.
- PASSPORT
Medicaid Waiver Program - Ohioans who cannot afford to pay for in-home care themselves may be eligible for PASSPORT, a Medicaid waiver program that provides a robust selection of care-managed services.
- Choices Medicaid Waiver Program - Choices, available in some parts of the state, is a Medicaid waiver program that puts the consumer in the driver seat, allowing him or her to guide the plan of care.
Assisted Living - Some folks may find that they just simply can't remain at home safely, but still do not require the around-the-clock care of a nursing home. For them, an assisted living facility may hold the answer.
- Assisted Living Medicaid Waiver Program - People on a Medicaid who need more help than in-home care can provide or no longer require nursing home care may be able to move into an assisted living facility, thanks to this new Medicaid waiver program.
- Residential State Supplement - This program offers a cash supplement to low-income older, blind or disabled persons, allowing them to live in a home-like group living arrangement within their own communities.
Comprehensive Care: Program for All-inclusive Care for the Elderly (PACE) - The Program of All-Inclusive Care for the Elderly (PACE), available in Cleveland and Cincinnati, provides Ohioans age 55 or older with primary and specialty care, adult day health services, personal care services, inpatient hospital, prescription drugs, occupational and physical therapies and nursing home care.
Nursing Homes - Ohio has nearly 1,000 nursing homes that provide excellent care to those who need it. But choosing the facility that is right for you or a loved one can be a daunting task. We can help take the guesswork out of it.
- Pre-Admission Review - All persons seeking admission to a Medicaid-certified nursing facility must undergo a pre-admission review to determine the most appropriate plan of care. A full range of institutional, home and community-based services are considered, when appropriate.
