Constituent Assistance

In most cases, constituents should be directed to their Area Agency on Aging for services they are eligible to receive. However, if the constituent's attempt to obtain assistance or to resolve a problem is unsuccessful, contact our Legislative Liaison. Alternately, you may submit the following form to request assistance on behalf of your constituent:

Please download the Release of Information Form (113kb), complete it and have the constituent sign. Then, fax it to this office.

The mail/fax form and Release of Information are in Adobe Portable Document Format and will require Adobe's free Acrobat Reader or equivalent software to view.
